Job Title: RN Trainer/Supervisor

Chautauqua Opportunities is seeking a highly skilled and experienced RN Trainer/Supervisor to join our team. As a key member of our Home Care department, you will be responsible for planning, developing, and implementing comprehensive training programs for our Home Care staff.

Key Responsibilities:
  • Develop and implement training programs for Home Care staff, utilizing the NYS approved Home Care Core Curriculum.
  • Provide direct supervision and evaluation of Home Care workers, ensuring compliance with DOH regulations.
  • Conduct on-the-job training and joint orientation for new personnel.
  • Maintain accurate patient files and employee health records.
  • Perform initial admission assessments and open cases as needed.
  • Collaborate with the scheduler to assign staff to patients and review patient care plans.
  • Provide case management and obtain physician orders as necessary.
  • Document and monitor accident/incident reports and facilitate quarterly QI meetings.
  • Maintain liaison with caseworkers, patients, families, and staff.
  • Ensure regulatory and procedural compliance.
Requirements:
  • Bachelor's degree in nursing required.
  • Current NYS license for Registered Professional Nurse.
  • Two years' experience working with patients, with three years preferred.
  • Two years' supervisory experience required, with three years preferred.
  • Ability to provide leadership and supervision.
  • Excellent writing and organizational skills.
  • Ability to develop training plans and problem-solve.
  • Customer service skills and basic computer skills.
Working Relationships:

You will work closely with our Health and Family Services Director, Home Care Clerk Scheduler, PCA I's, PCA II's, and other external partners to provide comprehensive care to our patients.

Organizational Responsibilities:
  • Adhere to all policies and procedures.
  • Attend in-services and trainings as required.
  • Participate in committee structures and data collection.
  • Ensure regulatory compliance and participate in organizational committee structures.
Physical Demands:

This role requires the ability to lift up to 25-50 pounds and work in a fast-paced environment.

Special Requirements:
  • Must be able to provide consistent, even-tempered customer service.
  • Complete all required NYS background clearances.
  • Ability to obtain and maintain physical exam and TB testing.
  • Valid driver's license and reliable transportation.
