Job Description

At The Camden Center, we are seeking a highly skilled and compassionate Case Manager to join our team. As a Case Manager, you will play a critical role in supporting our clients and their mentors/companions in achieving their goals and improving their overall well-being.

Key Responsibilities:
  • Client Support: Provide regular sessions with clients to discuss their progress, goals, and challenges, and make adjustments to their care plans as needed.
  • Communication: Maintain open and effective communication with clients, mentors/companions, family members, clinical providers, non-clinical providers, and financial guarantors to ensure seamless coordination of care.
  • Case Management: Develop and revise client reintegration plans, provide phone support, and document all interactions and discussions with team members.
  • Medication Management: Collaborate with mentors/companions to manage medication and ensure timely administration.
  • Toxicology Screens: Manage all information related to toxicology screens, including urine samples.

Requirements: To be successful in this role, you will need excellent communication and interpersonal skills, a strong understanding of case management principles, and the ability to work effectively in a team environment.
