Job Description

We are seeking an experienced LPN to provide administrative and clinical support to our teams within home care. In this role, you will interact with patients to ensure initiation and coordination of homecare services, manage clinical concerns and symptoms, and support the virtual implementation and clinical follow-up of the plan of care.

Responsibilities:
  • Interact with patients to ensure initiation and coordination of homecare services, manage clinical concerns and symptoms, and support the virtual implementation and clinical follow-up of the plan of care.
  • Answer phones in a timely manner, manage/handle patient requests, and route appropriately, enlisting management assistance when necessary.
  • Schedule patient appointments via phone by determining reason for visit, following established schedules and protocols, using appropriate billing area/appointment location, communicating changes, and confirming appointments.
Requirements

To be successful in this role, you must have an LPN-PA or LPN-NJ license, as well as high school diploma/GED and graduate of an approved Practical Nursing Program. You should also have clinical healthcare experience with significant interactions with patients, clinicians, and/or customers.
