Critical Care Transition Nurse

**Job Summary:**
We are seeking a highly skilled Critical Care Transition Nurse to join our team in Augusta, Georgia. This is an exciting opportunity to provide specialized care for patients transitioning from critical care to general medical-surgical units.
Key Responsibilities:
- Assess and monitor patients' vital signs, cardiac rhythms, and respiratory status in the step-down unit.
- Administer medications, perform procedures, and collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Educate patients and families on post-intensive care recovery, medication management, and self-care techniques.
- Communicate effectively with healthcare team members to facilitate patient progress and optimize outcomes in the step-down unit.
Requirements:
- Current RN license
- BLS (Other Certifications as required by facility)
- Two years of recent Step Down experience
- Strong communication and adaptability skills
