About the Role

We are seeking a highly skilled and compassionate Patient Care Coordinator to join our team at CrossWinds Counseling & Wellness. As a Patient Care Coordinator, you will play a vital role in providing care coordination services and assisting clients through our Same Day Access or scheduled intake process.

Key Responsibilities
  • Provide Care Coordination Services: Achieve expected referral rates for internal and external programs, monitor assigned CMHC & CCBHC outcomes, and coordinate with internal and external providers to increase frequency of desired outcomes.
  • Build Effective Relationships: Build professional relationships with clients and families, emphasizing client strengths, choice, and empowerment. Assessments and client interactions are delivered in a trauma-informed and respectful manner with integration of cultural competency across diverse populations.
  • Meet Productivity Standards: Achieve or exceed hourly productivity standards as outlined by supervisor, and achieve or exceed target numbers of CMHC & CCBHC data collection requirements, including outcome measures.
  • Communicate Effectively: Ensure that key client information is communicated to direct service providers and support staff prior to and after appointments, and confirm that clients have all required information to complete next steps in treatment prior to leaving the building after initial appointments.
  • Facilitate Collateral Communication: Facilitate communication between clients, support staff, direct care providers, and the community to assist clients in meeting all health-related goals, with an emphasis on behavioral health.
  • Maintain Positive Relationships: Develop and maintain positive working relationships with all resources within and outside the agency, and participate actively and timely in all administrative and treatment staffing.
  • Enter Data Accurately: Enter CMHC & CCBHC data into all required systems within assigned timelines and requirements of the CCBHC model, and document services provided and client responses in a timely, current, and accurate manner.
  • Update Client Information: Update client assessments and other information in a timely manner, and ensure that notes and other communications are sufficient to keep all team members informed of client contacts, significant events, and follow-up steps that may be needed.
  • Conduct Initial Intake Interviews: Conduct initial in-person client intake interviews with new clients, and ensure that all needed demographic, financial, insurance, and basic health and family history data is obtained from the individual.
  • Open New Client Charts: Open new client charts at the start of mental health treatment and evaluation services through the Intake process.
  • Provide Quality Customer Service: Provide quality customer service to clients, staff, and community partners.
  • Assist with Treatment Plan Updates: Assist with the coordination of treatment plan updates and other targeted case management tasks for the county.
Work Environment

This role operates in a professional office setting, and the employee is occasionally required to stand, walk, sit, handle objects, grasp and reach for items with hands and arms, manipulate hands and fingers, climb stairs, balance, stoop, kneel, or crouch, and talk or hear. The employee must occasionally lift or move up to 15 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Language skills are required, including the ability to read, write, comprehend, and speak the English language.
