Key Responsibilities:
  • Develop and implement individualized care plans that meet patients' unique needs.
  • Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
  • Communicate effectively with patients, families, and caregivers to address concerns and provide education.
  • Monitor patient progress and adjust care plans as necessary.


Requirements and Qualifications:
  • Degree in nursing from an accredited institution.
  • Illinois RN license and current BLS certification.
  • Excellent communication and interpersonal skills.
  • Able to work effectively in a fast-paced environment.


Compensation: $80,000 - $100,000 per year.
