Job Summary: The Clinical Care Coordinator plays a vital role in ensuring the efficient movement of patients through the hospital system. Reporting to the Manager of Case Management and Care Transitions, this position coordinates a plan of care to achieve clinical outcomes and promotes safe and timely discharges.

About the Role: As a member of the clinical team, the Clinical Care Coordinator works under the guidance of the Nurse Case Manager Team Leader to manage the plan of care throughout the care continuum. This requires expertise in discharge planning, resource utilization, and case management.

Salary and Benefits: The estimated salary for this position is $85,000 - $105,000 per year, depending on experience. Additionally, Medical University of South Carolina offers a comprehensive benefits package, including health insurance, retirement plans, and paid time off.

Required Skills and Qualifications: To be successful in this role, candidates must have a Bachelor's degree in Nursing from an accredited school of nursing and three years of work experience as a Registered Nurse. Prior leadership experience and case management certification are preferred.

Work Environment: The Clinical Care Coordinator will work in a fast-paced hospital setting, interacting with patients, families, and healthcare teams. A high level of physical stamina is required, as the position involves standing, walking, and lifting up to 50 pounds.
