The Home Care Assistant plays a crucial role in our organization, offering essential assistance to participants while actively engaging in the care planning process as a key member of our Interdisciplinary Team (IDT).

We are committed to delivering outstanding care for our participants and fostering a positive work atmosphere for our staff.

Key Responsibilities:

The Home Care Assistant will be responsible for delivering personal care and support with daily living activities (ADLs) to participants in their residences. This includes, but is not limited to:

  • Assisting participants with Instrumental Activities of Daily Living (IADLs), such as laundry, light housekeeping, and shopping.
  • Measuring and documenting vital signs as per instructions.
  • Helping with light meal preparation and ensuring participants are nourished and hydrated.
  • Facilitating safe transfers in and out of homes and providing medical escort services to appointments as required.
  • Supporting the Rehabilitation department by implementing home exercise programs as outlined in the care plan, following adequate training.

Additionally, translation services may be provided as needed, contingent upon qualifications.
