Key Responsibilities:

Implement individualized care plans for clients and document completed tasks. Aid clients with personal care tasks (bathing, dressing, etc.). Provide or assist with routine household cleaning, meal preparation, and laundry services. Transport clients to medical appointments, grocery stores, and other essential errands. Support clients in the self-administration of medications. Monitor and report any changes in the client's health status. Uphold a high level of confidentiality at all times due to access to sensitive information. Maintain regular, predictable attendance and demonstrate flexibility to meet departmental needs. Adhere to all Medicare, Medicaid, and HIPAA regulations and requirements. Comply with all organizational policies, procedures, and standards. Perform additional duties as assigned.

Position Requirements & Competencies:

Must be at least 18 years old. Successful completion of the Agency qualifying examination. Must pass a criminal background check. CPR / First Aid certification as required by state or program. Compassionate and nurturing disposition with a genuine desire to care for others.
