Key Responsibilities- Collaborate with healthcare providers and the interdisciplinary team to develop and implement comprehensive care plans for residents.
- Provide physical and psychosocial support to residents and their families, focusing on their unique needs and preferences.
- Perform clinical duties in accordance with state nurse practice acts and nursing standards of practice.
- Monitor resident clinical and care conditions, identifying changes in status and taking action to ensure patient comfort and safety.
- Work with care associates to provide routine nursing care to assigned residents as needed.
- Develop and coordinate holistic plans of care to meet residents' clinical and care needs.
- Support residents' preferences, routines, and choices in their electronic medical records.
- Ensure accurate and timely documentation of resident care.
- Participate in care conferences for designated residents as requested by Clinical Manager.
