Disability Care Coordinator

We are currently seeking a skilled Disability Care Coordinator to join our team in Martinsville, VA. As a key member of our residential waiver group home team, you will work closely with individuals with intellectual and developmental disabilities to provide support and guidance in their daily lives.
This is a full-time position that requires a high school diploma/GED, as well as at least one year of documented residential experience working directly with individuals with ID/DD or other developmental disabilities. Knowledge of group home policies and effective communication skills are also essential.
Responsibilities:
- Provide individualized support and training to residents in areas such as independent living skills, personal care skills, housekeeping skills, and recreational/leisure activities
- Collaborate with QDDP to ensure the program operates effectively and efficiently
- Document resident progress and maintain accurate records
