Job OverviewWe are seeking a passionate and dedicated Registered Nurse (RN) to join our team as a Shared Living Oversight specialist. In this role, the RN will provide oversight and support for shared living homes, working closely with individuals receiving services, their families, care providers, and service partners.
Key Responsibilities- Intake and Assessments: Complete intake and assessments for new referrals and as required thereafter.
- Supervision, Training, and Evaluation: Provide supervision, training, and evaluation of caregivers.
- On-Site Visits and Monitoring: Conduct on-site visits with each member per determined need and more often as needed.
- Communication and Teamwork: Report changes in health status of any members to the member's physician, your Director, team members, and other pertinent parties.
- Documentation and Discharge Planning: Complete nursing progress notes at each visit and fulfill other requirements per service type.
- Educational Information and Training: Provide ongoing educational information and training to all members & caregivers on health and aging.
- On-Call System Participation: Participate in the on-call system as assigned.
- Current MA Registered Nurse License.
- Two years of recent experience working with elderly or disabled adults, or individuals diagnosed with ID/D, ABI, or chronic conditions affecting their ability to live safely in the community.
- Background Check Required.
- Valid driver's license with an excellent RMV record (subject to RMV background check).
- Computer and organizational planning.
- Strong interpersonal and communication skills.
- Ability to work collaboratively in a team environment.
