Key Responsibilities- Clinical Responsibilities
- Develop and maintain therapeutic relationships with clients, utilizing concepts of reflection and motivational interviewing.
- Recruit and enroll a caseload of active clients, ensuring adherence to Colorado caseload targets.
- Continuously monitor and support client engagement in the program, implementing strategies to retain clients.
- Ensure professional boundaries and adhere to confidentiality requirements.
- Formulation and Implementation of Plans
- Conduct comprehensive and holistic nursing assessments, including model-specific assessments.
- Formulate nursing diagnoses and develop plans of care with clients, focusing on mutually accepted health goals and objectives.
- Deliver nursing services through home visits, case management, and community outreach programs.
- Documentation and Quality Improvement
- Document health history, assessments, goals, and objectives in compliance with department and program policies.
- Participate in review and analysis of ETP reports, identifying achievements and areas for improvement.
- Contribute to quality improvement efforts, ensuring adherence to professional standards and best practices.
- Reflective Practice and Collaboration
- Attend and participate in weekly reflective/clinical supervision sessions with NFP supervisors.
- Engage in ongoing learning and professional development to meet NFP Nurse Home Visitor competency requirements.
- Collaborate with colleagues and community agencies to provide comprehensive services and support to clients.

- Perform work in a way that is equitably inclusive and effective within diverse communities.

- A bachelor's degree in nursing from an accredited college or university.
- A minimum of one year of nursing experience, preferably in adult, pediatric, and community-based services.
- A Colorado Nursing license and current CPR certification.
- Up-to-date immunization status, including COVID.
- A valid driver's license and access to dependable transportation for business purposes.
- Proficiency in use of online meeting platforms and Microsoft Office tools, including Word, Excel, PowerPoint, and Outlook.
- Manual dexterity for performing exams and completing charts.
- Frequent and continual driving, walking, sitting, standing, bending/stooping, twisting, kneeling, and squatting.
- Frequent light lifting/carrying of equipment/supplies and infants (under 15 lbs.), and occasional to frequent moderate lifting/carrying of infants and small children.
- Sufficient hearing and speech ability to communicate in person and via phone to respond to inquiries and present information to individuals and groups.
- Reading comprehension to understand client records and medical documents.
