Job Summary

A Case Manager at Vision House provides critical support and advocacy to individuals and families in need, helping them overcome barriers and achieve their goals. This role requires a strong understanding of social services, excellent communication skills, and a passion for making a positive impact in the community.

Responsibilities
  • Provide case management services to clients, including assessing needs, developing plans, and connecting them with community resources.
  • Work collaboratively with a team to ensure seamless delivery of services and achieve program goals.
  • Develop and maintain relationships with community partners, including social service agencies, healthcare providers, and educational institutions.
  • Conduct crisis interventions and provide resources and advocacy to clients in need.
  • Update client databases and case notes in a timely manner, ensuring accurate and comprehensive records.
  • Participate in regular supervision and staff meetings to ensure alignment with program goals and objectives.
Requirements
  • Bachelor's degree in Social Work or related field.
  • Two years of experience in social services, including direct client work and case management.
  • Strong knowledge of social services, including assessment, planning, and resource development.
  • Excellent communication and interpersonal skills, with ability to work effectively with diverse populations.
  • Ability to work independently and as part of a team, with strong problem-solving and critical thinking skills.
