Job Summary

We are seeking a highly motivated and compassionate Care Specialist to join our team. As a Care Specialist, you will play a vital role in delivering high-quality care to our patients, working closely with our clinical team to ensure seamless coordination and support.

Key Responsibilities:
  • Conduct direct outreach to patients via phone and in-person to enroll them in our program and collect key data about their needs.
  • Meet patients in their homes or in the community to conduct needs assessments, set health goals, and facilitate in-home provider appointments.
  • Support patients in meeting their healthcare goals, working within an interdisciplinary team to ensure seamless coordination and care.
  • Document patient information accurately and efficiently, using electronic medical records to ensure continuity of care.
  • Build strong relationships with patients, families, and caregivers, using motivational interviewing techniques to engage and empower individuals.
Requirements:
  • At least 2 years of relevant work experience in a healthcare setting, preferably in chronic care management or working with chronically ill/elderly patients.
  • High school graduate or GED required; associate's or bachelor's degree in a healthcare-related field preferred.
  • Valid driver's license and auto liability insurance; reliable transportation and ability to travel within assigned territory.
  • Experience with electronic medical records and basic computer skills, including typing.
  • Multi-lingual capabilities a plus, but not required.
