Key Responsibilities- Conduct comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
- Develop and implement personalized treatment plans in consultation with physicians and revise as necessary.
- Collaborate with the interdisciplinary team to develop and implement client care plans.
- Treat patients to relieve pain, develop or restore functional abilities, and maintain maximum performance using physical means.
- Teach, supervise, and counsel patients and their families on physical therapy programs and adaptive devices.
- Record and report evaluation data, treatments, and patient responses to therapy interventions.
- Prepare evaluations of patients' conditions and perform periodic reassessments.
- Supervise Physical Therapist Assistants and Home Health Aides as required.
- Graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
- Licensed or registered by the State.
- At least one year of institutional experience.
- Home care experience and supervisory experience preferred.
- Ability to drive and access clients' homes, which may not be routinely wheelchair accessible.
- Hearing, eyesight, and physical dexterity sufficient to perform physical assessments and client care.
- Flexible scheduling.
