- Conducts clinical evaluations and implements treatment plans at acute, restorative, and maintenance levels for program participants.
- Evaluates participant conditions, formulates treatment strategies, and collaborates with participants or their caregivers on therapy execution.
- Integrates therapy plans into the overall Plan of Care (POC) established by the team.
- Facilitates the ordering, management, and upkeep of durable medical equipment for participants.
- Conducts home visits and educational sessions as necessary, documenting participant and caregiver responses in their records.
- Educates participants and their families on proper care techniques, exercise regimens, and the use of durable medical equipment (DME).
- Implements treatment programs based on interdisciplinary team (IDT) assessments.
- Instructs family members and caregivers on maximizing safety and functional abilities in daily living activities, ensuring documentation in medical records.
- Monitors participant progress and communicates findings to relevant staff.
- Documents all services rendered and maintains departmental records in compliance with documentation standards.
- Conducts data collection, prepares reports, and maintains necessary participant statistics.
- Contributes to the development of policies and procedures related to occupational therapy.
- Assists healthcare providers in assessing functional levels, aiding in the development and adjustment of care plans as needed.
- Participates in training programs and presents educational sessions as required.
- Collaborates with healthcare professionals in patient care planning.
- Executes all prescribed occupational therapy procedures as directed by healthcare providers.
- Prepares reports for healthcare providers regarding participant progress.
- Attends scheduled staff meetings and patient care discussions.
- Completes discharge summaries upon conclusion of therapy services.
- Submits evaluations, clinical notes, and progress reports promptly as per agency policy.
- Reports any incidents or variances immediately.
- Engages in peer review and quality management activities as requested.
- Participates in the review of medical records as assigned.
- Adheres to agency standards, accurately interpreting and executing all assigned duties.
- Reports any incomplete tasks to the appropriate clinical management personnel.
- Collaborates with InnovAge nursing and assisted living staff regarding the occupational therapy needs of their residents.
- Conducts and participates in educational programs within the clinic.
- Maintains a clean and organized therapy department and oversees therapy equipment maintenance.
- Ensures compliance with quality assurance, safety, environmental, and infection control protocols.
- Meets documentation requirements and maintains records of OT services provided as established by InnovAge programs.
- Participates in interdisciplinary team meetings to formulate Plans of Care for InnovAge PACE program participants.
Required:
- Graduation from an accredited Occupational Therapy program recognized by the Committee on Allied Health Education and Accreditation.
- Bachelor's, Master's, or Doctorate in Occupational Therapy.
- National Certification by the American Occupational Therapy Association or eligibility for the National Registration Examination.
- Registration with the relevant state authority.
- Possession of a valid driver's license, personal transportation, and a good driving record.
- A minimum of one year of experience working with elderly or frail populations.
- Two years of experience in the field is preferred.
- At least one year of general occupational therapy experience in a geriatric setting is preferred.
