We are seeking a highly skilled and compassionate Addiction Medicine Provider to join our team at Horizon Services. As a key member of our healthcare team, you will play a vital role in providing high-quality care to individuals struggling with substance use disorders.

About the Role

As an Addiction Medicine Provider, you will be responsible for providing initial assessments, medication management, and follow-up services to individuals admitted to our substance use, rehabilitation, and recovery programs. You will also participate in clinical case conferences and case reviews to provide consultation and guidance to our team.

Key Responsibilities:

  • Provide initial addiction medicine assessments and follow-up medication management services
  • Participate in clinical case conferences and case reviews to provide consultation and guidance
  • Prescribe appropriate medication for substance use disorders
  • Communicate with primary counselors to ensure seamless care coordination
  • Ensure confidentiality and maintain accurate records

Requirements

To be considered for this role, you must have:

  • NP or PA licensure in New York state
  • 2 years of experience in addiction medicine or a related field
