Job Overview

SUMMARY: Collaborates closely with Housing Specialists to facilitate successful transitions for residents into permanent housing for a minimum duration of one year. Responsible for maintaining accurate records, conducting follow-up communications, and performing local outreach visits. Employs a comprehensive approach to evaluate progress and identify client requirements. Links clients to available benefits through the DCF Access Program.

KEY RESPONSIBILITIES:

  • Evaluate participants for follow-up services and establish personalized objectives.
  • Monitor the progress of out-placed residents on a monthly basis, offering support and intervening when required.
  • Guide and oversee volunteer mentors while preparing reports for Chapman Partnership.
  • Maintain case documentation, organize Follow-Up Celebrations, and assist with transportation logistics.
  • Execute additional tasks as assigned to support program goals.

REQUIREMENTS:

  • Bachelor's degree in social services or a related discipline; CAP certification is a plus.
  • A minimum of three years of experience in case management within the social services sector.
  • Possession of a valid driver's license and a clean driving record.

SKILLS AND COMPETENCIES:

  • Understanding of social services, counseling, and behavioral sciences.
  • Ability to assess individual situations, empower residents, and collaborate effectively with others.
  • Experience working with vulnerable populations, excellent communication skills, and proficiency in social service management.
  • Bilingual proficiency in English and Spanish and/or English and Creole is preferred.
