About the Role

We are seeking a highly skilled and motivated Physical Therapy Assistant to join our team of dedicated healthcare professionals. As a Physical Therapy Assistant, you will play a vital role in providing high-quality patient care and collaborating with our interdisciplinary team to achieve optimal health outcomes.

Responsibilities
  • Collaborate with the interdisciplinary team, patients, families, and primary care providers to develop holistic plans of care focused on achieving physical therapy goals.
  • Instruct, coach, guide, and intervene to support patients and families in achieving best possible outcomes while remaining in their valued communities.
  • Promote optimal health, well-being, and quality of life of home health patients and their families by providing skilled, intermittent physical therapy in the patient's home.
  • Practice independently in the patient's home using excellent, evidence-based practice for assessment, clinical decision making, clinical practice, and coordination of care across the continuum.
  • Act as case manager and team lead for patients requiring home health therapy services, when nursing is either not involved in the case or physical therapy is the primary patient need.
  • Assess patients and collaborate with the interdisciplinary team and primary care provider to develop patient-centered plans of care.
  • Provide physical therapy treatment to patients/families/caregivers as ordered.
  • Complete accurate clinical documentation to effectively support service provided within specified timeframes.
  • Provide physical therapy to pediatric, adolescent, adult, and geriatric patients based on principles of growth and development and life stages.
  • Provide instruction to patients/caregivers and revise treatment plans as appropriate.
  • Maintain communication regarding patients with the case manager and other disciplines via care conferences or other appropriate methods.
  • Report acute medical problems and condition changes to the primary care provider.
  • Provide supervision to Physical Therapy Assistants, Home Health Aides, and students in the clinical setting.
  • Complete regulatory required assessments within specified timeframes.
  • Complete medication reconciliation at specified timeframes.
Requirements
  • Must meet all state educational requirements.
  • 1-2 years outpatient rehabilitation setting.
  • At least two (2) years of physical therapy experience highly preferred.
  • One (1) year experience in home health or other health setting with geriatric emphasis.
  • Current license in Physical Therapy or ability to obtain prior to hire.
  • American Heart Association Healthcare Provider BLS preferred upon hire, and required within 90 days of hire.
  • Current Driver?s license.
  • Excellent customer service skills.
  • Ability to work without direct supervision.
  • Provides care appropriate to the age of patients served on the job.
  • Knowledge of physical therapy assessment and treatment methods.
  • Knowledge of physical therapy modalities and equipment.
  • Ability to maintain effective relationships with other healthcare professionals.
  • Must be able to develop skills in use of technology used in home care setting.
  • Demonstrate the highest level of professionalism when interacting with patients, families, physicians, and hospital staff members.
Compensation

The estimated salary range for this position is $65,000-$80,000 per year, depending on experience and qualifications.
