Key Responsibilities

As a Direct Support Professional, you will provide direct support and assistance to individuals in their daily lives, helping them to achieve their goals and live independently.

  • Provide Direct Support: Assist individuals with socialization, health maintenance, medication administration, and skill or behavioral development.
  • Record Progress: Accurately document progress and activity, reviewing records and logs to stay up-to-date on changes in service plans.
  • Manage Assets: Assist with money management and ensure the safety of individuals' funds and property.
  • Maintain Relationships: Develop and maintain healthy and professional relationships with individuals, their families, and case managers.
  • Report Incidents: Report any instances of alleged abuse or neglect, medical, behavioral, or other incidents in accordance with Sevita policy and external requirements.
Health Care

As a Direct Support Professional, you may be required to:

  • Accompany Individuals: Escort individuals to medical appointments and relay orders and information to and from medical providers as required.
  • Administer Medications: Accurately administer and document delivery of medications and treatments, promptly reporting any administration errors.
  • Monitor Health: Monitor individuals' health, document health concerns, and communicate with nurses or supervisors as necessary.
  • Ensure Safety: Participate in safety drills and protect individuals in the event of an emergency.
Employment Responsibilities

As a Direct Support Professional at Sevita, you will:

  • Attend Training: Participate in orientation and ongoing training as directed and attend monthly staff meetings.
  • Work Scheduled Hours: Work scheduled hours, arrive on time, and stay on shift until a replacement arrives.
  • Teamwork: Work collaboratively with others, valuing their contributions and maintaining a respectful and courteous attitude.
  • Workplace Safety: Comply with all established safety policies, procedures, and rules, reporting any hazards or safety concerns to your supervisor.
Requirements

To be successful in this role, you will need:

  • Education: A high school diploma or equivalent, with six months of experience in human services preferred.
  • Certifications: A valid driver's license in good standing and completion of all state-required training and certification.
  • Physical Requirements: Heavy work, exerting up to 100 pounds of force occasionally, and/or up to 50 pounds of force frequently, and/or up to 20 pounds of force constantly to move objects.
