Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at American Health Communities. As an LPN, you will play a vital role in providing high-quality care to our residents.
Key Responsibilities- Direct the day-to-day functions of nursing assistants and other staff being supervised
- Provide direct nursing care to residents, including administering medications and treatments as ordered by physicians
- Conduct regular assessments of residents' conditions and document observations, interventions, and outcomes
- Supervise feeding of residents as necessary and ensure that physician orders are followed
- Notify physicians and other healthcare professionals of changes in residents' conditions
- Perform administrative duties, such as documenting nurses' notes, assessments, and physician orders
- Chart pertinent facts and information in a professional, clear, and comprehensive manner
- Admit, transfer, and discharge residents according to policy
- Recognize and respond to changes in residents' conditions and document observations, interventions, and outcomes
- Order medications and treatments from the pharmacy and prepare and administer as ordered
- Ensure that physician orders are signed off on and documented
- Conduct and/or monitor prescribed treatments, such as dressing changes, drawing of blood, and catheter changes
- Give and receive condition reports from on-coming and off-going shifts
- Perform emergency procedures, such as cardiopulmonary resuscitation, and notify family and other parties in the event of death
- Recognize, intervene, report, and document accidents and incidents when they occur
- Monitor and update care plans on each shift as appropriate
- Supervise and participate in fire safety and other disaster drills and be prepared to carry out duties in the event of an actual event
- Recognize, intervene, and/or report complaints and grievances from residents, families, visitors, and employees
- Monitor the facility for possible health and safety hazards
- Maintain privacy and confidentiality of records, conditions, and other information relating to residents, employees, and the facility
- Follow established universal precautions and isolation procedures
- Update daily census report and submit as required
- Attend and participate in orientation programs, on-going training, and educational classes
- Check inventory and request equipment, supplies, etc.
- Assist in updates of written policies and procedures of routine nursing care
- Participate in surveys made by authorized government agencies
- Monitor and participate in daily clean-up of the nurses' station
- Perform other related duties as assigned by supervisor
- Graduate of an accredited School of Nursing for Licensed Practical Nurses
- Current Practical Nurse licensure
- Current CPR Certification
- CPR Certification
The LPN will work in a skilled care setting, providing care to residents with a variety of medical conditions. The work environment is fast-paced and requires strong communication and interpersonal skills.
Physical RequirementsThe LPN will be required to stand, walk, sit, bend, reach, kneel, push, and pull frequently. The job will require frequent lifting of objects up to 40 pounds.
