We are seeking a skilled and compassionate Home Health Licensed Practical Nurse (LPN) to join our team at Interim HealthCare. As a Home Health LPN, you will provide high-quality patient care in the comfort of their own homes, working under the supervision of a Registered Nurse (RN).

Key Responsibilities:

  • Provide home-based care to patients with various medical conditions, including illnesses, injuries, and chronic diseases.
  • Work collaboratively with a multidisciplinary team, including physicians, RNs, and Certified Nursing Assistants (CNAs), to deliver comprehensive patient care.
  • Conduct patient assessments, document progress, and report changes to the patient's physician.
  • Educate patients and their families on their plan of care, medication, therapy, diet, and exercise.
  • Coach family members on patient care, medication, and home safety.
  • Collaborate with the interdisciplinary team to coordinate patient care and ensure goals are met and plan for discharge from services.

Requirements:

  • Graduate of a practical (vocational) nursing program for LPN and current GA LPN license.
  • Minimum of 1 year of experience in home healthcare.
  • HCHB charting experience highly preferred.
  • CPR certification, negative ppd screening or chest x-ray.
  • Drivers license, reliable transportation, and auto insurance.
  • Knowledge of state and federal home health regulations.
  • Clinical proficiency in nursing specialty, strong communication skills, and compassion.
  • Able to pass federal and state required criminal/abuse background checks and drug screen.
