Neogen Care is a leading provider of medical and non-medical home care services, and we are looking for a dedicated Home Health Therapist to join our team. As a Home Health Therapist, you will work closely with our clinical team to provide personalized care to patients in their homes.
Key Responsibilities:
- Develop and implement conditioning/rehabilitation programs consistent with the physician's Plan of Treatment and the overall goals of the patient/rehab team.
- Monitor the appropriate completion of documentation by physical therapy assistants and home health aides/personal care workers.
- Report patient progress to the patient's physician, Clinical Manager, staff, patient, and family.
Requirements:
- Graduate of an APTA accredited school of physical therapy.
- Successful completion of Certification Examination.
- 1 year of verifiable, supervised professional experience within the last 3 years.
- Valid PTA license in the state of practice.
- Current CPR/BLS certification.
- Compliant with state healthcare regulatory requirements.
- Possess up-to-date knowledge of treatment practices and physical therapy programs.
