Role Summary:
The LVN will work under the supervision of the Program/Clinic Manager and the clinical direction of a Registered Nurse (RN). This position is vital in providing direct nursing care to patients, ensuring their overall health and wellness.
Primary Responsibilities:
- Accurately process and execute medication orders following clinic protocols.
- Administer medications as directed by the medical team.
- Deliver high-quality nursing care and treatments to patients in the Day Health Clinic.
- Maintain an organized and well-stocked supply of clinic materials.
- Actively participate in the interdisciplinary team by contributing nursing insights during care planning and problem-solving.
- Educate and support patients, caregivers, and family members regarding nursing care and related matters.
- Perform additional duties as required to enhance patient care.
Qualifications:
- Completion of an accredited LVN program.
- Current LVN licensure in California.
- Demonstrated experience and interest in working with elderly or frail populations, with a minimum of one year in a similar role.
- Self-driven with the ability to work independently and collaboratively within a multidisciplinary environment.
- Bilingual abilities may be advantageous for specific roles.
