About This Role

We are seeking a dedicated Case Manager to join our team at Mass General Brigham.

The successful candidate will possess strong clinical and organizational skills, with the ability to work effectively in a fast-paced environment.

As a Case Manager, you will be responsible for coordinating patient care services along the continuum of care, ensuring efficient and effective delivery of services.

Key Responsibilities
  • Evaluating and predicting patient care trajectories
  • Facilitating collaboration between healthcare teams
  • Ensuring timely and effective communication with patients and families

Our ideal candidate will have a solid understanding of clinical care and systems management, with experience working in a similar role. A bachelor's degree in nursing or related field is required, with a current RN license in Massachusetts.
