We are seeking a highly skilled and compassionate Registered Nurse to join our team at Hudson Headwaters Health Network.
Job Summary:The Registered Nurse will be responsible for delivering high-quality patient care, utilizing the nursing process to assess, diagnose, plan, implement, and evaluate patient care. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
Key Responsibilities:- Provide direct and indirect patient care that is safe, ethical, patient- and family-centered, culturally congruent, and evidence-based.
- Collaborate with staff and other departments to develop strategies to continuously improve quality of care and the patient experience.
- Utilize data to meet care coordination needs across settings, in conjunction with the patient, family, and/or other members of the care team.
- Prepare patients for encounters by following established protocols.
- Protect patients and employees by adhering to infection control policies and protocols.
- Reconcile medication lists and prepare prescription renewals.
- Document patient information accurately and timely in the electronic medical record.
- Respond to patient phone calls using symptom-based policies and protocols to classify urgency and advise intervention options.
- Triage walk-in patients and assist with medical emergencies within the center.
- Initiate patient and family education plans that meet the needs of the patient.
- Perform point-of-care testing, including specimen collection and proper documentation.
- Assist with patient encounter flow and troubleshoot issues in collaboration with the supervisor.
- Provide direction to LPNs and Medical Assistants.
- Participate in meetings and serve on various organization committees.
- Assist with compliance and staff development duties.
- Unrestricted current RN license to practice nursing in New York State.
- A.A.S. degree or higher from an accredited school of nursing.
- Minimum of one-year Registered Nurse clinical experience preferred.
- Ability to travel throughout the Network.
- Current BLS certification.
- Proficiency with Microsoft applications and electronic medical records.
- Self-directed, detail-oriented, motivated, and ability to work independently as well as in a team atmosphere.
- Excellent verbal and written communication skills.
The pay rate for this position is $36/hour.
