Community Health Network is seeking an experienced Home Care RN Case Manager to join our team. As a Home Care RN Case Manager, you will be responsible for managing and caring for a caseload of patients and their families in the community, from admission to discharge.
Responsibilities:
- Effectively manage and care for a caseload of patients and their families in the community
- Supervise LPNs and Home Health Aides
- Conduct OASIS visits with a Medicare certified agency
- Develop and implement individualized care plans for patients
- Collaborate with healthcare teams to ensure high-quality patient care
Requirements:
- Minimum 1 year of recent Home Care RN Case Management experience
- Experience with OASIS visits and Medicare certified agencies
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
