We are seeking a skilled Speech Language Pathologist to join our team at VNA Health Group. As a Home Health Speech Language Pathologist, you will be responsible for providing high-quality speech and language therapy services to patients in their homes or community settings.

Key Responsibilities
  • Conduct comprehensive assessments and evaluations to identify speech and language therapy needs
  • Develop and implement individualized plans of care to address patient goals and objectives
  • Provide direct therapy services to patients, including speech and language therapy, to improve communication skills and overall quality of life
  • Collaborate with healthcare teams to ensure seamless care coordination and communication
  • Document patient progress and outcomes in a timely and accurate manner
Requirements
  • Master's degree in Speech-Language Pathology
  • CCC-SLP certification
  • 2-3 years of prior clinical experience and community-based experience
  • Strong computer skills and ability to navigate electronic medical records
  • Valid NJ driver's license
