Responsibilities
  • Implementation of Physicians' Prescribed Plan of Care: Deliver high-quality care to patients.
  • Patient Assessments and Coordination of Care: Ensure seamless care transitions.
  • Administration of Prescribed Medication, Treatments, and Therapies: Provide safe and effective care.
  • Compliant, Timely Documentation of Care Provided in System of Record: Maintain accurate records.
  • Health Promotion, Teaching, and Training of Family Members: Educate patients and families.
  • Responsible for Accepting and Writing Verbal Orders from Physicians as Required and for Obtaining Co-Signature of Case Manager on All Verbal Orders: Communicate effectively with healthcare teams.
  • Follow-Up with Physician, Client, Family Members, Staff, and Case Manager on Client Problems, Concerns, and/or Changes in Client Condition: Ensure continuity of care.
  • Performs Other Appropriate Duties as Assigned: Adapt to changing needs and priorities.
Qualifications
  • Must Have and Maintain an Active, Unencumbered License (RN/LPN/LVN) in the State in Which the Clinician Will Practice: Meet state-specific licensing requirements.
  • Current CPR Certification from American Red Cross, American Heart Association, or BLS: Demonstrate proficiency in CPR.
  • TB Skin Test (Current Within Last 12 Months): Meet TB testing requirements.
  • Six Months Prior Hands-On Nursing Experience Preferred but Not Required: Prior nursing experience is beneficial but not necessary.
  • Must Have Reliable Transportation: Ensure reliable transportation to patient locations.
