Job Description

This role involves providing high-quality patient care as part of the Patient Aligned Care Team (PACT). As a Licensed Practical Nurse, you will work closely with patients, families, and healthcare professionals to deliver exceptional care. Your duties will include assessing patient needs, developing individualized care plans, and implementing treatments under the guidance of licensed physicians and nurses.

Key Responsibilities:
  • Provide direct patient care, including administering medications, performing medical procedures, and documenting patient information
  • Collaborate with other healthcare professionals to develop and implement comprehensive care plans
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
  • Maintain accurate and detailed records of patient interactions, treatments, and outcomes
Requirements

To be considered for this role, you must meet the following requirements:

  • CURRENT UNRESTRICTED LICENSE TO PRACTICE AS A LICENSED PRACTICAL OR VOCATIONAL NURSE IN THE STATE WHERE YOU WILL BE WORKING
  • COMPLETED AT LEAST 1 YEAR OF QUALIFYING EXPERIENCE AS AN LPN/LVN
  • GS-5 LEVEL: COMPLETED AT LEAST 1 YEAR OF QUALIFYING EXPERIENCE AT THE GS-4 LEVEL OR EQUIVALENT AND FULLY MEETING ALL PERFORMANCE REQUIREMENTS FOR THE GS-5 LPN
