We are seeking a skilled Hospice Palliative Care Nurse Practitioner to join our team at MaineHealth. As a key member of our care team, you will work autonomously but in collaboration with physicians to deliver primary medical care and intraepisodic care for hospice patients.
Key Responsibilities:- Manage the terminal condition of patients and develop treatment plans to meet the hospice mission.
- Provide advanced practice nursing services through management of the terminal condition and developing treatment plans for patients at home and in the hospice house.
- Collaborate with physicians and other healthcare professionals to ensure comprehensive care for patients.
- Masters degree in nursing.
- State of Maine licensure as a Nurse Practitioner.
- Current certification in Basic Life Support for Healthcare providers.
- Previous experience and education in Hospice.
- Proficient with Electronic Health Record, OASIS, and PPS.
