Job Summary

Key Responsibilities
  • Provide clinical advice and guidance to patients and healthcare providers through phone and electronic correspondence, utilizing clinical judgment and triage protocols in accordance with regulatory requirements and standards of practice.
  • Develop and implement patient-specific education plans, focusing on preventive care, chronic disease management, acute diagnosis, medications, and other areas relevant to patient outcomes.
  • Prepare patients for visits by utilizing appropriate rooming standards tools.
  • Administer injections and medications as prescribed by providers, adhering to department protocols, regulatory requirements, and standards of practice.
  • Assist providers with various procedures, minor surgeries, and exams as designated.
  • Perform Point of Care Testing, including hearing, flu, urine, and strep tests.
  • Collect and prepare blood or other laboratory specimens for further testing.
  • Authorize prescription refills as instructed by providers.
  • Assist with test tracking and monitoring overdue results.
  • Remove sutures and change dressings on wounds as necessary.
  • Ensure exam rooms are adequately supplied and cleaned after each patient visit, maintaining and cleaning medical instruments and equipment after procedures.
  • Assist providers and patients in reviewing and revising care plans, assessing patient progress, and following up between visits as needed.
  • Participate in proactive outreach for patients requiring preventive and chronic disease services.
  • Perform care coordination activities among specialists and transitions of care to obtain necessary patient information.
  • Assist in coordinating referrals, education, and community resources.
  • Actively participate in performance improvement initiatives to enhance patient experience and quality of care.
  • Participate in daily huddles with providers.
  • Serve as a patient experience advocate, promoting a positive influence when interacting with patients, visitors, and colleagues, communicating professionally while prioritizing customer service.
  • Handle patient information in a confidential manner, adhering to Lancaster General Health's privacy and confidentiality policies and procedures.
Requirements
  • Completion of a LPN or RN educational program.
  • Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN), issued by the PA State Board of Nursing.
  • AHA CPR/BLS certification required within 30 days of hire.
  • Occasional regional travel may be required, necessitating a valid driver's license.
Preferred Qualifications
  • One to Two years of nursing experience in a primary or specialty care physicians' office.
  • Prior triage, emergency room, cardiac care, intensive care, or other related specialty nursing experience.
