Job Summary

We are looking for a dedicated and compassionate Physical Therapist to provide high-quality rehabilitation services to our patients in their homes and communities.

Responsibilities
  • Conduct comprehensive assessments of patients to determine their needs and develop personalized treatment plans.
  • Deliver rehabilitation therapy to patients in their homes or care facilities, using evidence-based practices and techniques.
  • Evaluate patient progress and adjust treatment plans as needed to ensure optimal outcomes.
  • Collaborate with interdisciplinary care teams to provide comprehensive care and support to patients.
Requirements
  • Minimum of one year of experience as a Physical Therapist or successful completion of a VNS Health Rehab Internship.
  • Licensure to practice as a Physical Therapist in New York State.
  • Valid driver's license required for all areas outside of Manhattan.
