RN - TeleNeuro Hospitalist Nurse Facilitator - Admit Discharge Transfer Resource Nurse

We are seeking a highly skilled and compassionate TeleNeuro Hospitalist Nurse Facilitator to join our team at Providence Medford Medical Center. As a key member of our interdisciplinary care team, you will play a vital role in facilitating patient progression and ensuring seamless transitions throughout the care continuum.
Key Responsibilities:- Designated TelePresenter nurse to assist the neurologist with non-emergent assessments for inpatient neurological follow-up appointments
- Communicate with the neurologist provider to determine appointment schedule for the day and inform direct care nurse about the appointment
- Moves the TeleNeuro video cart from patient room to the next patient room
- Facilitate patient progression by coordinating discharge planning and related tasks earlier in the patient's stay
- Collaborate with the patient's interdisciplinary care team to address barriers to discharge and support the patient throughout the discharge process
- Float throughout the house to complete discharge-related tasks for patients projected to be discharged that day or the next day
- Provide discharge education to patients and their families
- Graduation from an accredited nursing program
- Oregon Registered Nurse License upon hire
- National Provider BLS - American Heart Association upon hire
- 1 year Nursing experience
