The OpportunityWe're seeking an experienced Nurse Practitioner to join our Phoenix, Arizona team. As a Senior Health Care Specialist, you'll deliver high-quality, person-centered care to complex patients, contribute to our value-based care model, and work collaboratively with a multidisciplinary team to achieve exceptional patient outcomes.
- Provide onsite primary care services to seniors in their communities
- Develop and execute detailed care plans tailored to each patient's unique needs
- Collaborate with our Medical Director to address emerging issues and shape policies that enhance day-to-day practice, culture, and clinical guidelines
- Conduct monthly Chronic Care Management (CCM) visits and review CCM documentation
- Lead regular team huddles and foster a collaborative environment within the clinical team
- Identify opportunities to work closely with senior living management and care teams to implement Pine Park care plans and improve patient results
- Nurse Practitioner with geriatrics experience
- Comfortable working in a fast-paced, dynamic environment
- Willingness to travel to various locations within Phoenix, AZ, and drive between partner communities as needed
- Strong orientation towards collaboration and teamwork; values input from all members of the clinical team
- Excellent written and verbal communication skills
- Ability to work effectively with tech-enabled care delivery tools to enhance patient care and team performance
