Your Role:

As a Senior Adult Counselor, your primary focus will be on delivering impactful services to our program participants.

Key duties include:

  • Providing therapeutic interventions in both individual and group formats
  • Creating, managing, and monitoring treatment plans for participants
  • Conducting assessments specific to the program
  • Updating client records in the Electronic Health Record system
  • Facilitating effective communication within the team
  • Supporting program operations
  • Assisting with discharge planning and referrals
  • Maintaining compliance with required training and meetings
  • Carrying out additional assigned responsibilities
Qualifications:
  • Master's Degree in a relevant social services field
  • Minimum of 1 year of experience working with the target demographic
  • Successful completion of a Level II Background clearance
