The Home Care Specialist II provides direct, holistic medical and non-medical support to clients and their families in a home environment. A genuine passion for engaging with clients and integrating them into the care process is essential for success. This position offers the opportunity to contribute to reducing hospital admissions, minimizing emergency room visits, enhancing patient outcomes, and improving overall patient and provider experiences.
Key Responsibilities of a Home Care Specialist II:
- Deliver essential client care services, including:
- Maintain a safe and clean living environment through household tasks such as:
- Assist with dietary needs of the client, including:
- Support clients in their daily activities and overall quality of life, including:
- Complete daily electronic visit verification (EVV) check-ins and care plan documentation.
- Assist with personal care tasks, including:
- Oral hygiene
- Bathing, including showering and bed baths
- Toileting assistance
- Personal grooming, including skin care and shaving
- Dressing assistance
- Support with medication administration
- Range-of-motion exercises
- Client transfers and positioning
- Vital sign monitoring
- Ostomy care as directed by the RN
- Weight measurement
- Intake and output tracking as instructed
- Basic dressing changes as directed
- Cleaning client equipment
- Documenting daily client activities and observations
- Submitting weekly reports on client condition
- Client laundry and bed maintenance
- Housekeeping tasks in client areas
- Dishwashing and kitchen cleanliness
- Garbage disposal at shift end
- Shopping for client needs when necessary
- Meal preparation according to dietary requirements
- Assisting with client feeding as needed
- Providing snacks and nourishment between meals
- Engaging in social activities with family and friends
- Accompanying clients to appointments and outings as appropriate
- High School Diploma or GED
- Required: Certification/Licensure/Training in the State of Ohio as a Home Health Aide, STNA, or MA-C. Certifications from a state participating in the multistate privilege to practice compact with Ohio
- Minimum of 3 years of experience in acute care or home care
- BLS and ACLS Certification
- Clinical Proficiency: The Home Care Specialist II must demonstrate strong clinical skills and the ability to prioritize tasks effectively. Critical thinking is essential for assisting with and implementing a patient's care plan.
- Effective Communication: Collaboration is key; the Home Care Specialist II must work well within a multidisciplinary team, requiring open and responsive communication to meet patient needs.
- Cultural Sensitivity: The Home Care Specialist II should be open to understanding diverse patient values and beliefs, acting as a liaison between clients and healthcare providers.
- Respect and Patience: This role demands a high level of respect for clients, requiring patience and understanding while establishing reasonable boundaries.
- Commitment to Confidentiality: Awareness of patient confidentiality is crucial, adhering to HIPAA and PHI guidelines.
- Ability to lift and carry equipment/supplies weighing up to 50 pounds.
