  • Attend staff meetings and educational in-services to stay current on best practices and industry developments.
  • Adhere to guidelines for patient care and discharge planning to ensure seamless transitions.
  • Ensure patient needs are met and individualized care plans are documented.
Qualifications
  • State Physical Therapy License
  • One year of home care physical therapy experience preferred
  • Ability to make sound clinical judgments
  • Proficiency in clinical skills and Microsoft Office
  • Valid driver's license and reliable transportation
