We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at The University of Kansas Health System. As a Geriatric Care Coordinator, you will play a vital role in coordinating patient care and promoting health and wellness in our ambulatory clinic.
Key Responsibilities- Coordinate patient care through the nursing process, collaborating with physicians and team members to ensure comprehensive care.
- Provide physical and psychological support to patients and their families, promoting health and wellness.
- Administer medications accurately and safely, observing and recording patient reactions and responses.
- Assist with preparation of supplies and equipment, and aid physicians or advance practice providers during treatment, examination, and testing.
- Collect, document, and deliver lab specimens, and obtain lab results and other reports as needed.
- Notify patients of test results and follow-up appointments, and refer to RN/MD as necessary.
- Respond to and screen telephone calls for referral to RN or physician.
- Coordinate pre-visit planning by reviewing the EMR, including patient history, and proactively order labs and other tests based on approved clinic protocols.
- Graduation from an accredited licensed practical nurse program.
- High School Graduate.
- Previous medical office experience.
- Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA).
- Licensed Practical Nurse (LPN) - Multi-State - State Board of Nursing in the state (Kansas and/or Missouri) dependent upon work location.
