Position Overview:
The Health Management Coordinator plays a vital role in our Community Health Care Management Team. By connecting individuals in need of ongoing Care Coordination, we facilitate their journey towards achieving their fullest potential. This role promotes preventative care to minimize unnecessary emergency department visits and hospitalizations while addressing various social determinants of health.
Key Responsibilities:
The Care Coordinator will establish a professional and trusting rapport with members and community providers to ensure effective coordination and collaboration of services that lead to positive health outcomes. Responsibilities include:
- Providing a comprehensive range of services to members and their families, including care coordination, advocacy, education, and referrals to community resources.
- Conducting home visits to gather information through a thorough assessment, which will inform the development of a tailored care plan.
- Assessing medical and behavioral health needs, substance use, daily living activities, socio-economic status, and housing conditions to understand the member's overall health landscape.
- Creating a person-centered care plan that integrates a wide array of services in collaboration with an interdisciplinary care team, aiming to support members in achieving optimal wellness and recovery.
Qualifications:
To be successful in this role, candidates should possess:
- A Bachelor's Degree in Human Services or a related field.
- A minimum of two years of experience working with individuals facing chronic medical conditions and behavioral health challenges; five years of experience is preferred.
Desired Skills:
The ideal candidate will demonstrate:
- Strong planning and organizational abilities, along with excellent communication skills for effective collaboration with various stakeholders.
- Competence in interviewing members to evaluate decision-making capabilities, coping strategies, and barriers to managing healthcare needs.
- Adherence to established policies and procedures as directed by the lead health home and relevant health authorities.
- The ability to work independently while prioritizing tasks to efficiently coordinate care plans, as well as the capacity to thrive in a team-oriented environment.
- A valid driver's license and access to a reliable vehicle that meets insurance requirements.
Compensation:
The pay range for this position is $28.15 per hour, based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be compensated within the base salary equivalent of the stated hourly rates.
