Join Our Team as a Weekend Registered Nurse

We are seeking a dedicated and compassionate Registered Nurse to join our team at Commonwealth Home Health. As a Weekend Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working a flexible schedule from Friday to Monday, 7am to 6pm.

Key Responsibilities:
  • Provide comprehensive nursing care to patients in their homes, as prescribed by their physicians.
  • Conduct initial and comprehensive nursing evaluations, develop individualized plans of care, and ensure timely and accurate documentation.
  • Collaborate with other healthcare professionals to achieve continuity of patient care and to ensure seamless transitions.
  • Ensure patients meet home health eligibility and medical necessity guidelines, as defined by payer sources.
Requirements:
  • Current RN licensure in the state of practice.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
