We are seeking a highly skilled and compassionate Mobility Specialist to join our team as a Home Health Physical Therapist in Pueblo, CO.

About the Role:

  • As a Mobility Specialist, you will provide personalized home-based physical therapy services to patients who require assistance with regaining strength, managing pain, and improving mobility.
  • You will work collaboratively with a multidisciplinary team of healthcare professionals to develop and implement customized care plans that meet the unique needs of each patient.
  • Your expertise will be invaluable in helping patients achieve optimal outcomes and improve their quality of life.

Key Responsibilities:

  • Conduct thorough assessments and evaluations to identify patients' strengths, weaknesses, and goals.
  • Develop and implement individualized treatment plans that address patients' physical limitations and functional deficits.
  • Provide evidence-based interventions, including exercises, manual therapies, and education, to promote patients' progress and independence.
  • Maintain accurate and detailed records of patient progress, including assessment results, treatment plans, and outcome measures.
  • Collaborate with other healthcare professionals, including physicians, nurses, and therapists, to ensure seamless care coordination and communication.

Requirements:

  • Degree from an accredited Physical Therapist program and active licensure in Colorado.
  • Minimum of three years of experience in physical therapy, preferably in a home health setting.
  • Strong clinical skills, excellent time management, and effective communication abilities.
  • Able to work independently and as part of a team to deliver high-quality patient care.
