About the Role

We are seeking a highly skilled MDS Coordinator to join our team at Providence Care Center. As a key member of our leadership team, you will play a critical role in ensuring the accuracy of our resident assessments and care plan optimization.

Key Responsibilities
  • Conduct and Coordinate Resident Assessments: Ensure the timely and accurate completion of resident assessments (MDS) in accordance with current rules, regulations, and guidelines.
  • Collaborate with the Interdisciplinary Team: Work closely with the healthcare team to develop individualized resident care plans that meet their unique needs.
  • Monitor and Document Resident Progress: Track and document resident progress and responses to care plans, ensuring that their needs are met and their goals are achieved.
  • Participate in Quality Assurance and Performance Improvement: Contribute to the Quality Assurance and Performance Improvement (QAPI) process, identifying areas for improvement and implementing changes to enhance resident care.
  • Communicate with Residents, Families, and Healthcare Providers: Effectively communicate with residents, families, and healthcare providers to coordinate optimal care delivery and ensure seamless transitions.
  • Stay Up-to-Date on MDS Regulations and Guidelines: Stay current on MDS regulations and guidelines, ensuring compliance and maintaining the highest standards of care.
