Job Title: Clinic LPN/RN

The Southeast Medical Group is seeking a skilled Clinic LPN/RN to join our team. As a key member of our clinical staff, you will provide high-quality patient care and support functions to patients, working within the scope of your LPN/RN license and under the direction of our supervising Providers.

Key Responsibilities:
  • Assist Providers and clinical staff in identifying high-risk patients and implementing band stratification in the electronic medical record.
  • Conduct evaluations and patient education nurse visits for chronic disease states, providing clear instructions on treatment, medications, self-management goals, and action plans.
  • Triage incoming calls and perform outbound calls regarding band stratification allocation.
  • Assist in reviewing patient supportive care at home, social factors, and concerns, including assessment of durable medical equipment, home health, or other resources that could be beneficial for the patient and their family.
  • Communicate with hospitals and skilled nursing facilities regarding inpatients, effectively conveying status to physicians and staff.
  • Access designated local hospital physician portals to retrieve medical records when necessary.
  • Assist Providers with communication to the Care Management Team and document in the electronic medical record when appropriate.
  • Participate in daily/weekly huddles and Care Coordination meetings, ensuring current patient information and status are up-to-date.
  • Perform clinic patient wound care on stable conditions, assessing and documenting appropriately.
  • Demonstrate the ability to multitask and work effectively with multidisciplinary teams and individuals.
  • Perform all nursing care duties within the scope of a Licensed Practical Nurse/Registered Nurse, including injections, procedures, phlebotomy, and diagnostic testing.
Requirements:
  • Licensed Practical Nurse/Registered Nurse with a current license in good standing to practice in the state of employment.
  • Georgia State Practical Nurse/Registered Nurse.
  • Current CPR certification.
  • Ability to work efficiently with electronic medical records.
  • Effective use of proper grammar, spelling, and medical terminology.
  • Ability to react calmly and effectively in emergency situations.
  • Two years of related LPN/RN experience in a physician's office, clinical, or hospital setting.
  • Current IV certification.
  • Effective listening and communication skills.
  • Infusion experience.
Preferred Qualifications:
  • Three or more years of related LPN/RN experience in a physician's office, clinical, or hospital setting.
  • Knowledge of managed care and referral processes.
  • Experience related to patient education.
  • X-ray experience.
  • Proficient knowledge of chronic diseases, especially COPD/asthma, CHF, IHD, and diabetes.
Physical and Mental Requirements:
  • Ability to lift up to 50 pounds.
  • Ability to push or pull heavy objects using up to 50 pounds of force.
  • Ability to sit for extended periods of time.
  • Ability to stand for extended periods of time.
  • Ability to use fine motor skills to operate office equipment and/or machinery.
  • Ability to receive and comprehend instructions verbally and/or in writing.
  • Ability to use logical reasoning for simple and complex problem-solving.
  • Ability to travel to multiple locations to support business needs as required.
