Primary Care Provider Position

**Salary:** $30-34/Hour
**Responsibilities:)
1. Provide direct patient care within the scope of licensure, including patient assessment, vaccinations, medications, and treatments.
2. Develop and implement personalized treatment plans for patients.
3. Collaborate with healthcare teams to deliver quality care and improve patient outcomes.
4. Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
5. Maintain accurate records and documentation in the EHR.
6. Participate in ongoing education and training to stay up-to-date on best practices and emerging trends in community health care.
7. Contribute to quality improvement initiatives and patient satisfaction efforts.
**Requirements:**
• A current Massachusetts Registered Nurse License is required.
• Basic Life Support (BLS) certification is necessary.
• Bilingual skills in Spanish or Portuguese are preferred but not mandatory.
• Recent COVID-19 booster and annual flu vaccination are required.
