Job Summary:We are looking for a compassionate and experienced Registered Nurse to provide high-quality patient care in a Hospice setting. As a Hospice Registered Nurse, you will work closely with the healthcare team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities:- Provide direct patient care, including administering medications, wound care, and other treatments as ordered by the physician.
- Conduct thorough assessments of patients' physical and emotional needs, and develop and implement individualized care plans to meet those needs.
- Collaborate with the healthcare team to ensure seamless care coordination and communication.
- Document patient progress and outcomes in accordance with regulatory requirements and company policies.
- Participate in ongoing education and training to stay current on best practices and industry developments.
- One to three years of experience as a Registered Nurse in a Hospice setting.
- Current RN licensure in the state of Connecticut.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Ability to lift, push, and pull up to 25 pounds.
- Paid Time Off (PTO) after 1560 hours.
