These services are provided in home settings or adult care/supervised living homes, offering essential support to those in need.

In addition, you'll perform clinical evaluations for individuals with serious mental illness (SMI), intellectual disability (ID), or related conditions (RC) who are seeking admission to nursing homes. These preadmission screening and resident review (PASRR) Level II assessments typically take place in nursing homes or hospital settings, ensuring that each person receives the appropriate care and support.

As a key member of our team, you will evaluate, determine, and recommend the next level of care to improve overall quality of life. This is a hybrid role, combining work from your home office with travel to evaluate patients in your local area, including the Manchester area. Travel expenses, all mileage and parking will be fully reimbursed.

Key Responsibilities:

  • Conduct clinical assessments of patients, including reviewing relevant records and consulting collateral sources, to ensure accurate evaluations and recommendations.
  • Recommend appropriate levels of care and service needs based on a comprehensive understanding of the patient's physical, behavioral, and psychological functioning.
  • Complete assessments on time and within the scope of practice, ensuring all contractual requirements are met with precision and professionalism.
  • Demonstrate strong typing and computer skills, with proficiency in Microsoft Office Suite, including Word, Outlook, Excel, PowerPoint, and Teams.
  • Ensure strict compliance with program workflows and regulatory guidelines.
  • Perform person-centered assessments, delivering meaningful and appropriate recommendations for levels of care to enhance patient outcomes.
  • Leverage deep clinical knowledge and experience with psychiatric disorders, as defined by the most current DSM, to provide expert care in New Hampshire.
  • Apply knowledge of standards of practice in mental health, intellectual and developmental disabilities, geriatrics, and other conditions requiring long-term care services and supports.
  • Collaborate effectively with healthcare professionals, individuals, families, and community resources to optimize patient care.
  • Communicate clearly and professionally through spoken, written, and electronic media to ensure seamless coordination and patient support.
  • Exhibit good attention to details, prioritize tasks efficiently, and complete assignments in a timely manner.
  • Demonstrate a solid understanding of Medicaid programs, with experience navigating similar healthcare projects.

Required Qualifications, Skills and Experience:

  • Must hold an active, unrestricted RN license with the ability to practice in New Hampshire.
  • Graduation from an accredited college with a degree in a relevant program of study.
  • Proficiency in medical record abstracting and a thorough understanding of medical record organization, terminology, and disease processes.
  • Strong clinical assessment and critical thinking skills, ensuring high-quality patient evaluations and recommendations.
  • Exceptional verbal and written communication skills, with the ability to convey complex information clearly and effectively.
  • Proven ability to work collaboratively in a team-oriented environment.
  • Flexibility and excellent organizational skills, with the ability to manage multiple tasks and adapt to changing priorities.
  • Proficient in Microsoft Office Suite and skilled in internet and web-based navigation for efficient work processes.
