**Job Description:**

This part-time Occupational Therapist role involves working with patients to develop and implement personalized treatment plans that address their physical, emotional, and cognitive needs. As an Occupational Therapist, you will conduct thorough assessments, deliver high-quality services, and collaborate with multidisciplinary teams to ensure seamless transitions between care settings.

**Key Responsibilities:**

* Develop and implement individualized treatment plans in accordance with patient needs and goals.

* Conduct thorough assessments and ongoing evaluations to monitor patient progress and adjust treatment plans as needed.

* Deliver high-quality occupational therapy services, promoting patient-centered outcomes and ensuring seamless transitions between care settings.

* Collaborate with multidisciplinary teams to coordinate care, share best practices, and maintain open communication.

* Maintain accurate records and documentation of patient interactions, progress, and outcomes.

**Requirements and Qualifications:**

* Master's or Doctoral degree in Occupational Therapy from an accredited program.

* Current licensure and registration to practice as an Occupational Therapist in the state of employment.

* Ability to function independently with minimal supervision while maintaining professional relationships with patients and families.

* Strong commitment to lifelong learning, embracing innovation, and adhering to Trinity Health's Code of Ethics.

* Minimum one year of experience or completion of a 6-8 week preceptorship program.

**Compensation and Benefits:**

The estimated annual salary for this position is $65,000 - $85,000, depending on location and experience. Benefits include medical, dental, and vision insurance, 401(k) retirement plan, paid time off, and opportunities for professional growth and development.
