Job Summary

CVS Health is seeking a dedicated and compassionate Health Coach Associate to join our team in Long Island, NY. As a key member of our healthcare team, you will play a vital role in helping patients achieve their health goals and improve their overall well-being.

Key Responsibilities
  • Assist in the collaborative process of assessment, planning, implementation, and evaluation to engage and educate members in obtaining or maintaining optimal health status.
  • Conduct assessments of members through the use of clinical tools and information/data review, and facilitate integration of services.
  • Utilize influencing/motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes to achieve optimum level of health.
  • Provide coaching, information, and support to empower members to make ongoing independent healthy lifestyle choices.
  • Monitor, evaluate, and document care in collaboration with registered nurses and members.
Requirements
  • Must reside in Long Island, NY, or within commuting distance.
  • 3 years of clinical experience required.
  • LPN/LVN license in New York with multi-state/compact privileges.
  • Certain vaccines and health screenings may be required.
Preferred Qualifications
  • Previous coaching experience.
  • Previous work with Medicare beneficiaries.
  • Bilingual - English/Spanish.
